FIRST STEP:
Download the EXCEL file named "MCAF Cost Sheet" to your computer from the link below (You will rename this file and attach it to the Grant Request Form in the Second Step)
Once Downloaded to your computer, you will have to click "Enable Editing" to fill it out.
SECOND STEP:
Click this link to go to the Grant Request Form. All instructions are on the form, including how to attach the Cost Sheet.
